Nova Scotia Health Authority Central Zone (formerly Capital Health) is the province`s largest provider of health services, serving the Halifax, Eastern Shore and West Hants regions in addition to providing specialist services to the rest of Nova Scotia and Atlantic Canada. Service is delivered through hospitals, health centres, and community-based programs and includes medical and surgical care, mental health care, community health programs, addiction prevention and treatment and environmental health services. Through commitment to quality care and patient safety, two web-based Medication Reconciliation (Med Rec) applications were developed to improve efficiency and reduce errors: Discharge Med Rec (DMR) and Transfer Med Rec (TMR).
