Through close collaboration with the Baycrest Centre for Geriatric Care’s programs such as the Terraces and Apotex, and with external organizations such as Villa Colombo, Humber River Regional Hospital, and North York General Hospital, the Acute Care Transition (ACT) Unit at Baycrest provides acute/sub-acute health care for clients who would otherwise receive emergency room services or be admitted to acute care. The Emergency Department Avoidance Through Baycrest’s Acute Care Transition Unit program therefore provides improved client-centered care, enhances patient flow, reduces the number of Alternate Level of Care (ALC) bed days, and significantly reduces costs. The program connects with community stakeholders and develops collaborative partnerships that facilitate smooth client transitions between different programs.
