On an annual basis, the Geriatric Program Assessment Teams (GPAT) receive approximately 2200 to 2500 referrals. Of those, geriatric assessments were completed on 72 to 75 percent. Clients are then connected with resources such as day hospitals, home care, and inpatient rehabilitation to stay in the community as long as possible. By maintaining clients in the community, there is a cost savings to the health care system.
