Montfort Hospital

Type of Practice

Fruit of Montfort’s mission and vision, the hospital’s participation program for patients and their families was launched in 2014. The program has opted for two corporate activities to give patients and their families a voice: setting up a patient and family council [the Conseil des patients et des proches (CPP)] and taking steps to involve patient-partners in the various organizational bodies, such as committees and working groups. This collaboration with patients and their families at multiple levels of the organization has rounded out the theoretical and clinical knowledge of the interprofessional and care facilitator teams with the experiential knowledge of patients and their families.

Some guidance is necessary to ensure the collaboration’s success, so the program includes mechanisms for recruiting, selecting, training and integrating patient-partners. These mechanisms are conducted by the patient-partners themselves, who have a hand in implementing the program throughout the process.
