In the summer of 2018, the Forensic Psychiatry Program at St. Joseph’s Healthcare Hamilton created a Patient Safety Task Force with a mandate to promote safety for all. The Patient Safety Task Force collaborates to uphold a safe environment for both patients and staff. Membership is made up of current inpatients representing each of the forensic units in hospital. Staff resources to the Patient Safety Task Force include the Forensic Safety Specialist and the Forensic Knowledge Translation Lead. The Patient Safety Task Force terms of reference include the following objectives: 1. To support initiatives that promotes a safe environment. 2. To provide a forum for discussion of safety issues by including a perspective from patients. 3. Collaborate and present ideas that reduce hazards in the environment. 4. Allow patient members to bring forward safety concerns from their home inpatient unit that warrant attention of the Safety Task Force. 5. To review processes or solutions and provide input to safety issues where appropriate.
