The Seniors Rapid Assessment Service is a unique partnership with the Emergency Department and demonstrates a change in approach to health care. Instead of being reactive, the service aims to be proactive in the delivery of service. While at times it is difficult to measure the impact of prevention, there is no doubt that keeping seniors safe and in the place they call home increases the quality of life of individuals in the community. The service aims to reduce frequency of falls and connect seniors with services in the community. This in turn impacts Emergency Department attendance rates and admission rates. It also improves patient outcomes and quality of life as independent living is an important measure for many seniors.
