Working in partnership with the Community Care Access Centre, the 'Steps to Home' program facilitates the safe and timely discharge of patients from acute care facilities. This initiative prepares patients and families for the day of discharge, so they are ready to leave and will not request to stay longer. The program features a booklet in which patients record their journey from the hospital to home. This step-by-step document uses red/yellow/green coding to indicate where the patient is on his or her journey. The coding is also displayed on the patients’ charts and in their rooms. As the patient meets the necessary milestones, the colour coding changes from red to yellow and eventually to green indicating they are ready to leave the facility. This enables patients, health teams, and family members to know how close a patient is to being released from the hospital so everyone is prepared on the day it happens. It is a useful tool to keep the patient and family on track.
