The Integrated Assisted Living Program (IALP) is a new initiative that provides assisted living services for seniors in their own homes. Services can be provided to more people without the delay of building infrastructure and facilities. IALP uses a neighbourhood support approach adapted to client needs. There is 24/7 personal support assistance (scheduled and on-call), and an enhanced role for Personal Support Workers to support the chronic disease management model of services. In addition to individual services, congregational meals are provided. Basic activation and recreation needs are met through access to appropriate services and programs. This initiative has improved independence and provided clients with a greater sense of reassurance as they realize that support is only a call away. Improved well-being is also facilitated by the recreation and activation programs and reduced social isolation. Finally, the initiative has enhanced system efficiencies and effectiveness and community partnerships.
