Enhanced Activation Therapy and Restorative Care is an exciting new model that helps the Brockville General Hospital do two things: assist patients in their efforts to return safely home, and at the same time, minimize ALC numbers that threaten accessibility to acute care beds. Gaps in services for the frail elderly were identified during consultations with community partners while implementing other Aging at Home strategies. The new model was developed to address these gaps. Access to services for the frail client requiring additional time to recover functional status is improved through this model and moves the individual and family both within sectors and across sectors to ensure that the client gets the right services at the right time as increasing or decreasing levels of care are required.
