RVH’s Simcoe Muskoka Regional Cancer Program (SMRCP) continues to experience increasing volumes of new patient referrals. Increased volumes emphasizes the importance of clinics operating efficiently and effectively. After a review of leading evidence and best practice, opportunities were recognized to improve the model of survivorship care at the SMRCP. This included improved utilization of space, human resource and most importantly, improved access to appropriate transitions in care for patients. To commence, a new model was implemented for well-breast patients.

The goals of this clinic are many:

1) facilitate transitions between the SMRCP and primary care providers (PCPs)

2) reduce wait-time for new patients to be seen by Oncologists

3) optimize utilization of space and human resources in the SMRCP with respect patients off active treatment and in remission

4) support patients with their physical and psychosocial needs

5) ensure patients feel safe and supported throughout their transition by seeing the appropriate provider, in the right place at the right time

Both patients and PCPs communicated that gaps exist in providing practitioners with updates on patients oncology care. To ensure patients have access to coordinated and interdisciplinary care services during the transition from active treatment to extended survival, primary care providers have to be informed of all aspects of survivorship care. A survivorship clinic facilitated by an oncology nurse practitioner that operates next door to the SMRCP provides patients with the medical, physical, emotional and psychosocial support they need before their care is transitioned from their oncologist to their PCP.
